Crisis Advocacy & Resource Education (CARE)
PROGRAM REQUIREMENTS
                                

General Emergency Services

How we help
- Provide resource information for city, state, and Federal programs
- Case management
- Bus passes
- Gas cards, car repairs
- Prescriptions
- Miscellaneous needs

Program requirements:
- Call Care Center to schedule an appointment with the Case Technician
- Eligible clients can receive these services once every 3 months

Bring with you to appointment:
- Form of picture identification
- Proof of all income & expenses (copies of all bills, pay stubs, etc)
- Proof of need (notice to quit, eviction or shut-off notice)

Medication Bridge Program

This program assists low-income individuals access medications directly from pharmaceutical companies.

Program requirements
- Client must be  a U.S. citizen
- Must have no other type of prescription coverage
- Income must be less than 20% of poverty level:
   family of 2, less than $24,240 /yr
- Only long-term medications are covered
- Call Care Center to make appointment

Bring with you to appointment:
- Form of picture identification
- Proof of all income & expenses (copies of all bills, pay stubs, etc)
- Proof of need

Dental Connection Referral

We serve as a referral agent for the Greater Nashua Dental Connection, Inc.

Program requirements:
- Must meet Federal eligibility guidelines – family of 2, less than $24,240/year.
- Must have ability to pay for dental services based on reduced Medicaid rates.
- Call Care Center to schedule an appointment with the Case Technician

Bring with you to appointment:
- Form of picture identification
- Proof of all income & expenses (copies of all bills, pay stubs, etc)
- Proof of need

IDA Program

This savings program offered through the NH Community Loan fund is designed to help low-income families.  The program matches 3/1 every dollar saved.

Program requirements:
- Must attend 12 hours of economic literacy and asset training annually
- Savings goals of home ownership, education or opening small business
- Must save between $25-$100 monthly from earned income
- Call Care Center to make appointment with Case Technician
